چکیده

Pulmonary incompetence is a common sequel of pulmonary valvotomy, but many authors believe that the haemodynamic consequences of this regurgitation are well tolerated (Talbert, Morrow, Collins, and Gilbert, 1963). In this paper we report a patient who had severe pulmonary incompetence after valvotomy and who failed to maintain her initial clinical improvement. Because of the severe leak it was considered reasonable to correct the incompetence surgically. A homograft pulmonary valve was used, and we are unaware of any previous report of its use in man, though Ross, Cooper, and Brockenbrough (1961) described the experimental replacement of the pulmonary valve with a tricuspid semilunar prosthesis in dogs.

Right ventricular Hemodynamic Alteration after Pulmonary Valve Replacement in Children with Congenital Heart Disease

Introduction:  In patients who underwent surgery to repair Tetralogy of Fallot, right ventricular dilation from pulmonary regurgitation may be result in right ventricular failure, arrhythmias and cardiac arrest. Hence, pulmonary valve replacement may be necessary to reduce right ventricular volume overload. The aim of present study was to assess the effects of pulmonary valve replacement on rig...

Mitral valve replacement with stored inverted pulmonary homograft valve.

The fate is described of 11 patients who had a stored pulmonary valve homograft mounted on a frame or in a Dacron jacket used for mitral valve replacement. Mitral regurgitation due to holes in the attenuated cusps occurred in nine by one year, requiring re-operation or causing death. Only one patient remains well with moderate mitral regurgitation after two years. Since January 1967, 118 patien...

Left Main Coronary Artery Compression following Melody Pulmonary Valve Implantation: Use of Impella Support as Rescue Therapy and Perioperative Challenges with ECMO

The purpose of this case is to describe the complex perioperative management of a 30-year-old woman with congenital heart disease and multiple resternotomies presenting with pulmonary homograft dysfunction and evaluation for percutaneous pulmonary valve replacement. Transvenous, transcatheter Melody valve placement caused left main coronary artery occlusion and cardiogenic shock. An Impella ven...

Clinical and Hemodynamic Studies in Patients

Debate continues regarding the long-term clinical and hemodynamic benefit of homograft replacement of the diseased mitral valve. The results of valve replacement with mitral homografts in the 120 patients who have had operation at the Stanford Medical Center from May 1967 to November 1970 are given. The operative mortality rate has been 5% and the late mortality rate 6%. Anticoagulants were sto...

Haemodynamic changes in patients with high pulmonary vascular resistance after mitral valve replacement. Comparative study between use of unstented aortic homograft valves and Starr-Edwards prostheses.

The clinical and haemodynamic responses to mitral valve replacement were studied in 36 patients with a pulmonary vascular resistance of4 units or above. In ig patients, I3 women and 6 men (mean age 49-7 years) Starr-Edwards prostheses were used. In I7 patients, 7 women and IO men (mean age 53 6 years) fresh unstented aortic homografts were inserted. In the latter group, 3 patients had, in addit...
